New Patient Leaflet
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill.
You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. All new patients are offered a health check with a member our nursing team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. This usually takes about 10 minutes.
A doctor will check all the information you have supplied and decide if your registration will be accepted. If for any reason your application is declined (this is a very rare occurrence) we will contact you to explain the reason.
Your registration will be with the practice as a whole rather than with a particular doctor. You will understand that all the doctors are busy and your preferred doctor may be fully booked on the day you wish to see them but we will always endeavour to give you an appointment to suit your needs.
If you move out of the practice area you will need to register with a new practice.
